-

A tragic incident unfolded as a young man, Patrick Liam Coyle, 27, from Wigan, was discovered deceased at a residence in Accrington merely days after pleading for assistance from medical professionals. The distressing turn of events occurred six days post his discharge from a mental health facility. During a two-day inquest at County Hall in Preston, it was disclosed that Patrick had fervently sought help in the days leading up to his demise.

Following his release, Patrick visited the A&E department three days later, imploring to be readmitted on March 7. Despite his plea, he was deemed safe for discharge after affirming that he had no intentions of self-harm. Tragically, he was not heard from after March 8 and was discovered lifeless two days later.

Kate Bisset, the Area Coroner, revealed that Patrick had expressed a strong desire to return to inpatient care, highlighting his struggles with mental health and alcohol addiction. He had exhibited signs of escalating mental distress post-discharge, engaging in excessive alcohol consumption and medication use. Despite his attempts to seek help, mental health professionals assessed him as safe for release during his A&E visit on March 7.

On the evening of March 8, mental health support workers visited Patrick at his residence, noting his denial of suicidal thoughts. Plans were put in place for ongoing support; however, Patrick’s lack of communication thereafter led to the tragic discovery of his passing on March 10. The circumstances surrounding his demise remain unclear due to his intoxicated state and deteriorating mental health prior to the incident.
